Your First Dental Visit
Your first dental visit promises to be a pleasant experience.
Making sound decisions about your dental care and oral health is an easy thing to do with the right preparation before hand. The better we communicate, the better we can care for you.

Please arrive 15 minutes before appointment. This will ensure enough time to fill out paperwork and give you an optional tour of our office. This will help us to remain on time. You may download these forms and fill them out prior to your appointment. This will allow us to serve you more effectively. |

If you have dental insurance, please remember to bring your insurance card with you. You may also email or fax all information prior to your appointment. |

Make a list of questions to ask our office, so you don’t forget anything on the day of your appointment. This includes any concerns you have, or dental problems you have been experiencing. |
Appointments
If, for some reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Appointments cancelled less than 24 hours before scheduled appointment time will incur a charge.
Insurances
Our goal is to maximize your insurance benefits. Our office accepts most dental insurance programs. We are a PPO for Aetna, Cigna, MetLife, United Concordia, and TDA. We are also a Premier for Delta. Please call our office if you have any questions. Our staff will help to determine your dental benefits.
Financial Assistance
Major credit cards, such as Visa and MasterCard, Carecredit, cash, and personal checks are accepted in our office.
A 10% accounting adjustment is allowed when fees in excess of $125.00 are paid in full by cash or check on the day that treatment is presented or in advance of scheduling the appointment.
